Abstract

<jats:p>&lt;p dir="ltr"&gt;The overall aim of this thesis was to identify, in collaboration with individuals with prediabetes and health care professionals, key components of an integrated person-centered prediabetes care suitable for implementation in primary health care.&lt;/p&gt;&lt;p dir="ltr"&gt;The thesis applied a human-centered design approach, engaging relevant stakeholders throughout the development process. The research explored current practices, experiences, and needs related to prediabetes care in primary health care, providing a foundation for model development.&lt;/p&gt;&lt;p dir="ltr"&gt;Findings demonstrated considerable variation in existing prediabetes care practices and highlighted challenges related to the identification, management, and follow-up of individuals with prediabetes. Health care professionals emphasized the need for clearer structures, increased prioritization of prediabetes, and improved strategies for integrating preventive care into routine practice. Individuals with prediabetes described the condition as existing between health and disease and expressed a need for personalized support to facilitate sustainable lifestyle changes and effective self-management.&lt;/p&gt;&lt;p dir="ltr"&gt;Through collaborative workshops involving both people with prediabetes and health care professionals, essential elements of a person-centered care model were identified. These included improved communication, structured care pathways, active stakeholder involvement, and support tailored to individual needs.&lt;/p&gt;&lt;p dir="ltr"&gt;Overall, the findings underscore the importance of strengthening prediabetes care through more consistent and person-centered approaches within primary health care. By providing structured, individualized support and follow-up, primary health care has the potential to contribute to the prevention of type 2 diabetes and promote better long-term health outcomes.
