Abstract

<jats:p>Knee osteoarthritis is a highly prevalent chronic non-communicable disease worldwide and remains a leading cause of disability, pain, and diminished quality of life. Its complex pathophysiology requires multimodal therapeutic strategies, among which therapeutic exercise is recognized as a cornerstone intervention. Exercise improves joint mobility, reduces pain, enhances muscular strength, and contributes to overall health outcomes. However, beyond these well-established benefits, less explored dimensions involve the integration of exercise with chronobiology and chronotherapy of articular cartilage. Recent studies indicate that circadian rhythms influence cartilage metabolism, inflammatory responses, and tissue repair, suggesting that the timing of exercise sessions may critically modulate therapeutic efficacy. Aligning rehabilitation protocols with biological rhythms could optimize patient response, enhance the sustainability of outcomes, and open new avenues for personalized musculoskeletal care. This perspective underscores the need for further research into Chrono therapeutic approaches to exercise in knee osteoarthritis, bridging clinical practice with molecular and temporal biology.</jats:p>
