Abstract

<jats:p>Neurosurgical therapy of movement disorders experienced a renaissance with the introduction of deep brain stimulation (DBS) in the 1980s. DBS aimed to improve symptoms in Parkinson’s disease, tremor, and dystonia, which are among the most important indications. With advances in brain MRI and the implementation of real-time intracerebral temperature monitoring, focused ultrasound has again expanded treatment options by broadening the neurosurgical therapeutic armamentarium. Having started DBS in 1999 and having contributed extensively to the scientific literature together with our neurological colleagues, we have, since 2020, also used MR-guided focused ultrasound (MRgFUS) to treat patients with tremor by targeting the ventral intermediate nucleus (VIM; nucleus ventralis intermedius) and patients with Parkinson’s disease by targeting the subthalamic nucleus (STN; nucleus subthalamicus). Differential indications for DBS and MRgFUS will be reviewed, and experiences from a high-volume center with more than 1,500 patients treated by either method will be described.</jats:p>
