Abstract
<jats:p>Bu kitap bölümü bir vaka analiziyle adölesan gebeliklerde tıbbi sosyal hizmet yaklaşımını kadın ruh sağlığı perspektifiyle ortaya koymayı amaçlamaktadır. Adölesan gebelik kadının fiziksel, ruhsal ve sosyal açılardan tam iyilik halini olumsuz etkileyen yapısal bir kadın refahı sorunudur. Bu nedenle “bireyi sosyal çevresi içinde değerlendirme” ilkesine dayalı biyopsikososyal yaklaşım ve güçlendirme temelli sosyal hizmet müdahalesinin önemi büyüktür. Vaka analizinde sosyal inceleme raporu ve derinlemesine görüşmelerden elde edilen veri, tematik olarak çözümlenmiş ve etik ilkeler doğrultusunda anonimleştirilmiştir. İncelenen vakada adölesan gebe ve aile sisteminde dikkat eksikliği ve hiperaktivite bozukluğu (DEHB), intihar girişimi öyküsü ve psikotik bozukluklar gibi ruhsal sorunların; damgalanma, güvenlik riski, bakım yükü ve okuldan uzaklaşma gibi kadın refahını etkileyen risklerin birlikte görüldüğü saptanmıştır. Müdahale sürecinde travma bilgili yaklaşım, kurumlar arası iş birliği ve toplum temelli bakımın koruyucu rolü somutlaştırılmış; eğitimde kalmanın desteklenmesi, ruh sağlığı izlemi ve güvenli sosyal destek ağlarının kurulması örneklendirilmiştir. Bölüm, adölesan gebeliklerde sosyal hizmetin kadın refahını merkeze alan işlevine yönelik uygulama temelli önerilerle sonuçlanmaktadır. This book chapter aims to present the medical social work approach to adolescent pregnancies through a single case analysis from the perspective of women’s welfare and women’s mental health. Adolescent pregnancy is considered a structural women’s welfare problem that adversely affects women’s complete well-being in physical, psychological, and social dimensions. Accordingly, the chapter demonstrates the practical implications of a biopsychosocial approach based on the principle of “assessing the individual within her social environment” and empowerment-oriented social work intervention. In the case analysis, data obtained from the social investigation report and in-depth interviews were thematically analyzed and anonymized in accordance with ethical principles. The examined case revealed the coexistence of mental health problems such as ADHD, a history of suicide attempt, and psychotic disorders within the adolescent pregnant woman and her family system, along with risks affecting women’s welfare, including stigma, safety concerns, caregiving burden, and disengagement from education. During the intervention process, the protective role of a trauma-informed approach, interagency collaboration, and community-based care was illustrated; practices supporting school retention, mental health follow-up, and the establishment of safe social support networks were exemplified. The chapter concludes with practice-oriented recommendations regarding the function of social work that places women’s welfare at the center in adolescent pregnancies.</jats:p>
