Abstract

<title>Abstract</title> <p>Background To evaluate changes in cortical lens density and anterior subcapsular lens density (ALD) after implantable collamer lens (ICL) implantation using IOL Master 700, and to analyze their clinical significance. Methods This prospective study included 41 patients (82 eyes) who underwent ICL implantation. High-resolution B-scan images were obtained using IOL Master 700. Cortical lens density and ALD were quantitatively analyzed using Image J software preoperatively and at 1 day, 1 week, 1 month, 3 months, 6 months, and 12 months postoperatively. Results Mean preoperative spherical equivalent was − 7.09 ± 2.27 D. Cortical densities at preoperative, 1 day, 1 week, 1 month, 3 months, 6 months, and 12 months were 90.7 ± 11.9, 97.5 ± 15.4, 97.2 ± 10.9, 95.7 ± 13.4, 92.1 ± 12.0, 92.2 ± 8.1, and 93.2 ± 8.4, respectively (F = 1.57, P = 0.16). ALD at the same time points were 100.6 ± 13.0, 106.5 ± 11.2, 106.3 ± 14.8, 104.9 ± 11.0, 105.0 ± 11.0, 105.1 ± 11.0, and 105.0 ± 11.1, respectively (F = 1.51, P = 0.17). Compared with preoperative values, ALD increased significantly at 1 day, 1 week, and 1 month postoperatively (P = 0.03, 0.03, and 0.02, respectively), with no significant changes at 3, 6, or 12 months (P &gt; 0.05). Conclusion In the early postoperative period (within 1 month) after ICL implantation, both cortical lens density and ALD show a transient increase but stabilize and return to preoperative levels by 3 months postoperatively, with no significant difference compared with baseline during one-year follow-up.</p>
